Please Note: Registration packets must be complete in order to reserve your child's spot in the preschool. All paperwork must be completely filled out and a current shot record must be attached to the paperwork along with the non-refundable fees. The non-refundable registration fee is $30 per child and is collected on registration day. The non-refundable supply fee for the 2 day program is $80 and the non-refundable supply fee for the 1 day program is $40. The supply fee is collected on registration day.
Registration Dates 2010-2011 School Year
Current Families:
Registration packets for our Monday/Wednesday families will be due no later than February 3rd.
Registration packets for our Tuesday/Thursday families will be due no later than February 4th.
Registration packets for our Friday Fun Day families will be due no later than February 12th.
New Church Families
Registration for PHUMC families who are members of the church but are new to the preschool will take place on February 22nd from 10:00 am - 1:00 pm.
New Families to the Preschool
Registration for families who are brand new to PHUMC Preschool will start on March 4th between 9:30 am - 12:30 pm and continue until all slots are filled. We recommend calling the school to check for class availability.
State of New Mexico Immunization Requirements and Exemption Form
As of January 4, 2010, the following immunizations are required for preschool:
DTaP 3-4(4th due at 48 months of age) Polio 3 MMR 1 (2 recommended after 4th birthday) HIB 3 Varicella 1 Hep A 1-2
