Registration for the 2010-2011 School Year

We still have a few spots available in our Pre-K classes for 2010-2011 and we have a wait list for all other classes.  Please call the school at (505) 899-0956 for more information regarding placing your child on our wait list.   Registration paperwork must be downloaded from this website and be completely filled out and turned in when you register. Click here to access the Registration packet.  Please also refer to the links at the bottom of the page for current State of New Mexico Immunization requirements.

PRESCHOOL PLAYGROUND AGREEMENT

If your child requires medication at school, you must also provide signed copies of the Prescription Medication Form or the Non-Prescription Medication Form.  

We must also have the signed Indemnity Agreement on file.

Please Note:  Registration packets must be complete in order to reserve your child's spot in the preschool.   All paperwork must be completely filled out and a current shot record must be attached to the paperwork along with the non-refundable fees.   The non-refundable registration fee is $30 per child and is collected on registration day.   The non-refundable supply fee for the 2 day program is $80 and the non-refundable supply fee for the 1 day program is $40.   The supply fee is collected on registration day.  

State of New Mexico Immunization Requirements and Exemption Form

As of January 4, 2010, the following immunizations are required for preschool:

DTaP 3-4(4th due at 48 months of age)
Polio 3
MMR 1 (2 recommended after 4th birthday)
HIB 3
Varicella 1
Hep A 1-2
