Registration for the 2012-20123School Year

We will be starting registration for the 2012-2013 school year in February.  Specific dates will be posted here as they are determined.  

Registration paperwork must be downloaded from this website and be completely filled out and turned in when you register.  Click here for the 2012-2013 Registration Packet.  Please also refer to the links at the bottom of the page for current State of New Mexico Immunization requirements.

PRESCHOOL PLAYGROUND AGREEMENT

If your child requires medication at school, you must also provide signed copies of the Prescription Medication Form or the Non-Prescription Medication Form.  

We must also have the signed Indemnity Agreement on file.

Please Note:  Registration packets must be complete in order to reserve your child's spot in the preschool.   All paperwork must be completely filled out and a current shot record must be attached to the paperwork along with the non-refundable registration and supply fees.  

State of New Mexico Immunization Requirements and Exemption Form

As of January 4, 2010, the following immunizations are required for preschool:

DTaP 3-4(4th due at 48 months of age)
Polio 3
MMR 1 (2 recommended after 4th birthday)
HIB 3
Varicella 1
Hep A 1-2
